What is this?

In partnership with UC Health, NeighborHub Health meets people where they are so they don't have to travel to a clinic to get help. This street-based care is designed for people experiencing homelessness who face barriers like transportation, stigma or fear of traditional settings. We focus on building trust over time so patients can get the medical, behavioral health and addiction care they choose, at their own pace.
